You are eligible to enroll into the health insurance after 60 days of full time employment (32 hours per week). After 60 days, you can make your benefit elections and will be enrolled on the first day of the following month.

HiveMind Construction will contribute $300 per month towards the cost of your health insurance! Any additional amounts are paid "above the line" pre-tax via payroll deductions.

The health insurance is through Independence Blue Cross (aka Keystone Health Plan East) AND one GOLD level option with Highmark Blue Shield. There are 3 plan options, the HMO Silver Proactive, and PPO Silver Classic, and the Highmark PPO Gold.

• The HMO Silver Proactive: HMO Proactive Network
  • Requires you to choose a primary care doctor who you'll get referrals from.
  • All local Philadelphia Blue Cross hospitals (and most doctors) are in-network but divided into tiers.
  • Costs vary by provider tier, with Tier 2 & 3 being more expensive than Tier 1, especially for costly services like hospitalization and surgery.
  • Copays for most procedures except surgeries & hospitalizations (unless you stay with a Tier 1 provider).
  • HMO Proactive Tiers
• The PPO Silver Classic: PPO Network
  • National coverage with a large network.
  • No referrals needed for specialists.
  • Most services have only a copay, while some higher-cost procedures involve a deductible and coinsurance (i.e. 30% of costs).
• HIGHMARK: PPO Gold: Highmark PPOBlue Network
  • National coverage with a large network.
  • No referrals needed for specialists.
  • Most services only have a copay, while some higher-cost procedures having a $1000 deductible and with full coverage after the deductible.

We participate in the US Federation of Worker Cooperatives aggregated Dental & Vision Plan.

The disability insurance provides coverage for ALL active employees working at least 30 hours per week. The coverage will replace the lessor of $1500 per week or up to 60% of income for up to 6 months. The coverage activates after 2 weeks, so the first 2 weeks of disability are not covered. The coverage can also provide "zero day residual" coverage in that a partial return to work MAY qualify for partial disability payments. Please see the benefit summary in the next tab for more details.

Our Accident Insurance is pays money directly to you in case of an accident occurs, regardless if other copays / costs were incurred or are lower than the Accident insurance payment. All employees with at least 3 years of service qualify for the company paid accident insurance. If you have not been employed for at least 3 years, you may enroll on a voluntary basis. You would pay the premiums directly to Colonial Insurance.
