To Enroll, click "Make Benefits Election".

The health insurance open enrollment occurs during the month of February and any changes are effective March 1. The other benefits operate on a calendar year basis with open enrollment between 11/15 and 12/15 with an effective date of January 1.

Any amounts that you will pay for health, dental, and vision benefits are via "above the line" pre-tax payroll deduction.

There are 2 plan options, the PPO Gold 500 and the PPO Platinum. Both are excellent plans.

• Highmark PPO Gold 500
  • The Gold plan has excellent coverage with low copays.
  • It is a PPO network so no referrals are required to see specialists (Make sure they are in-network before going).
  • $500deductible for major procedures, but no copay for most procedures after the dedutible is met.
• Highmark PPO Platinum
  • The Platinum plan has excellent coverage with low copays.
  • It is a PPO network so no referrals are required to see specialists (Make sure they are in-network before going).
  • No deductible and generally low copays for all procedures.

You are eligible to enroll into the 401K plan after 1 year of employment with 1000 hours of service during the previous year.

The plan is a "Pooled Employer Plan" sponsored by Fellow Travelers. The 401K plan is "shared" by many companies to keep costs & fees low.

You will automatically be enrolled (into a target date fund) if you are eligible with a 3% salary deferral if you do not make an election. This election will INCREASE by 1% each year unless you make an election. The company will match the first 4% of your 401K salary deferrals, dollar for dollar.

Our dental insurance is exceptional! It effectively eliminates network restrictions by reimbursing out-of-network visits at in-network copay and coinsurance rates in almost all cases. However, we encourage using in-network providers whenever possible, as higher in-network usage helps secure better renewal rates for the future.

Our insurance is provided through Humana and offers excellent benefits. The vision insurance is especially valuable for those who regularly use glasses or contacts. The pre-tax premiums are likely lower than your annual vision care expenses.

This vision insurance can be used on top of the vision benefits with the health insurance. You can be double reimbursed for the same contacts / lenses. You should use the Humana insurance first, then request the reimbursement from health insurance vision coverage after.
