Register for unitedeservices to access temporary ID cards, claims info, and more! If you don't receive a card in the mail within 14 business days, let us know because something went wrong!
The dental insurance is with Ameritas & the vision is with Eyemed... both through the US Federation of Worker Cooperatives Group plans.
Health Insurance
Am I Eligible & How Do I Enroll?
You are eligible to enroll into the health insurance after 60 days of full time employment (40 hours per week). After 60 days, you can make your benefit elections and will be enrolled on the first day of the following month.

Opportunity Threads will pay for 80% of your premium if you are a Co-op member, 60% for Associates, and 50% if you are not a Co-op or Associate member. You will pay for the cost of adding any dependents to the plan.

What is the Insurance?
The health insurance is through United Healthcare and is a HMO plan. There is also dental & vision insurance available as well through the US Federation of Worker Cooperatives Benefit Association. You also automatically receive life insurance paid for by the company.
The health insurance requires a deductible be met for less common services. Doctor visits, simple diagnostics, prescription drugs, & urgent care visits are covered BEFORE the deductible met and is only a copay. Higher cost but less frequent events such as hospitalizations, MRI, and outpatient surgeries require the deductible be met before the insurance begins to cover some of the costs. PLEASE REMEMBER TO ENROLL INTO THE CARE CASH PROGRAM! It's free money that can be used to pay for medical expenses.

401K Retirement Plan
You are eligible to enroll into the 401K plan after 1 year of employment with 1000 hours of service during the previous year.
The plan is a "Pooled Employer Plan" sponsored by Fellow Travelers in, a Professional Employer Organization (PEO). The 401K plan is "shared" by many companies to keep costs & fees low.
You will automatically be enrolled (into a target date fund) if you are eligible with a 2% salary deferral if you do not make an election.
The company will match the first 4% of your 401K salary deferrals, dollar for dollar.

The Dental Insurance is fantastic! There is virtually no network because if you see an out of network provider, the dental plan will reimburse the out of network visit at in-network benefit copays / coinsurance! Only in very rare situations would that not be the case (VERY RARE).
